Our resources help to explain and define available end of life choices. They support and are supported by our public policy projects – which aim to expand access to end of life choices in the US and internationally.
The Completed Life Initiative Medical Aid-in-Dying Map is intended to serve as a resource for patients and their families about eligibility requirements, important safeguards, and further information about how to access aid-in-dying in the United States. The Completed Life Initiative has several Advance Directive forms for New York State residents that you can download. Including: Healthcare Proxy form, Do Not Resuscitate (DNR) Order, Living Will, Dementia Directive, MOLST, and POLST forms.
There are many forces that differentiate us in the world: politics, religion, beliefs to name only a few. One thing that unites us is the desire to die with dignity. As a result of these forces not all of us have the opportunity to do so, yet a great many fight to make a death with dignity possible for themselves and those they love. It is these people that were in the spotlight on World Right to Die Day, November 2nd, in 2022; the Champions, who through their support, activism, fighting, writing, willpower, vision and strength, are advocating to make right to die legislation possible all over the world.
